Nokomis Healthy Seniors

Nokomis Healthy Seniors is a Living at Home/Block Nurse Program serving the greater Nokomis area of Minneapolis. We help neighborhood seniors to continue living safely and independently in their homes by:

  • Providing information and referral to community services
  • Assisting in the coordination of services
  • Recruiting and coordinating volunteers to provide services
  • Providing nursing case management/health care coordination
